Picture Identification
All applicants and co-applicants must submit a copy of their government-issued picture ID. Below are two examples but other options, such as U.S. Passports or State ID cards, will also be accepted.
Tax Forms
We need the three most recent years of tax information for the applicant and co-applicant (if they have filed).
1040 Example
Schedule C (Form 1040) – if self-employed
Schedule 1 (Form 1040) – if self-employed
W-2s and 1099s
You will need to submit the three most recent years of W-2s and 1099s from any employment or assistance received by the applicant and co-applicant.
Verification of Income
Every family member over the age of 18 that is employed must submit 4 months (16 weeks) of their most recent pay stubs. Any adults in your household without income must fill out a zero income verification form, below.

Income from Assistance
All assistance that is received by members of the household must be documented. This includes TANF, SSI, SSDI, and Disability.
Verification of Other Income
If any member of the household receives alimony, child support, or regular income received from parents or relatives, please provide the following:
• Most recent 6-month history of payments
• A notarized letter (or similar agreement) to verify how much you receive and how often you receive the payments
